Chalkwise is an independent project. It is not affiliated with, endorsed by or connected to the Department of Education or any other government agency. References to DepEd, its forms or its orders only describe the formats and rules our tools are designed around.

Tools, not official forms

Our forms and reports follow commonly used layouts, but they are not the official documents. Formats and rules change, and schools and divisions sometimes ask for their own versions. When your school or division requires an official form, use that form. Please check our output against the latest orders before you submit it.

You check the results

The tools compute grades, averages, percentages, ratings and totals from what you enter. Mistakes in your entries, grading settings or school rules will carry through to the results. You are responsible for checking every figure before you print, sign or submit it.

AI suggestions can be wrong

Drafts and suggestions, including those from AI help and the “Suggest” buttons, are starting points. They may be inaccurate, generic or unsuitable for your class, subject or curriculum. Review and edit them. You decide what goes into your final document.

No professional advice

Nothing on this site is legal, financial, human resources or other professional advice. Memos, notices, letters and certificates are templates. Have them reviewed when the situation calls for it.

Your data and backups

Most records are saved in your browser on your own device. They can be lost if you clear your browser data, change devices or reset your phone. Save backup files regularly. We are not able to recover records that are stored only on your device.

Sample data

Names, numbers and examples shown in previews and templates, such as “Learner A”, are made up to show how a tool works. They do not describe real people or schools.

Availability and formatting

We work to keep the site available, but we do not promise that it will always be running or free of errors. Printed and Word output can look different on different devices, browsers and printers. Preview before you print.

We may link to websites we do not control. We are not responsible for their content or their privacy practices.
